If you live within our catchment area (please see below for further details) and wish to join our practice as a new patient, please complete our registration form.

Please return the completed form to the surgery.

Please fully complete every section of the registration form, if any information is missing we will contact you and this will lead to delays in your registration.

When you return the form, please make sure you inform our team if you have any medication on repeat issue, as you will need an appointment with one of our GPs to arrange the medication to be available on your repeat template. Please allow sufficient time for processing as we wouldn't want you to run out of your medication.

On receipt of the registration form we will inform you of the name of your accountable Doctor. Even though you will have an accountable Doctor, you are welcome to see any of our GPs at the surgery on your future visits.

Communicating with you - it is very important that we can communicate well with our patients. We want to make sure you can read and understand the information we send you. Please let us know if you find it hard to read our letters or if you need someone to support you at appointments, or if you need any of the following:

  • information in braille, large print or easy read
  • a British Sign Language interpreter or advocate
  • support to lipread or use a hearing aid or communication tool

If you have any special communication needs please inform the receptionist when you register or when you arrive for your next appointment.

registrationWe can accept new patients who live in Wool, Bovington, East Burton, East Stoke, Coombe Keynes, East and West Lulworth, East Knighton, Winfrith Newburgh, East and West Chaldon and Owermoigne.

We cannot accept registrations from Wareham; Crossways; Bere Regis; Tincleton, or Holworth and beyond

If you move, however nearby, please always notify us of your new address and phone number and check if you can remain registered. If you are moving within the inner boundary on the map please use the change of information form to let us know your new details.

Currently registered patients who move from within the inner boundary into our outer boundary may remain registered with the practice.

If you are under the care of the hospital please remember to let them know your change of address.

Please note we cannot take new patients who live within our outer boundary.
