Travel Immunisations

Please collect a travel questionnaire from the surgery or download below and hand in at reception once completed.  Travel clinics take place on a Friday afternoon, so your request will be dealt with the Friday following submission of your questionnaire, when the Practice Nurse will contact you to arrange an appointment, which will be between 3.30 and 5.50 p.m. on a Friday. The Practice Nurse can provide a wide range of travel health advice, including malaria prevention and prophylaxis.

This service is only available if we receive your travel health questionnaire 6 weeks prior to the date of your departure.

Please note that your fully completed travel assessment form must reach the GP Nurses at least six weeks prior to the date of your travel.  Any later than this may mean we are unable to offer you a service.  Also please give us as much information as possible on the form.

 

The Friday following receipt of your completed travel assessment form, or eight weeks prior to travel, the nurse will assess the risks for your, based on the information you have provided, your medical records, and her research.  She will then email you detailed information about your travel risks, in order for you to make an informed decision as to which vaccinations, if any, you feel that you need.  If you have no email address, she will telephone you to let you know the information is ready for you to collect from the surgery reception desk, at your convenience.  Certain courses of vaccinations require a period of at least a month to complete.  Some vaccinations have to be ordered and paid for prior to you receiving them.

 

If you are unable to complete the travel assessment form six weeks before travel, there is a private travel clinic at Boots the Chemist, in Poole, who may be able to help at short notice.
